Broken Instrument Retrieval
Sometimes during root canal treatment — whether at our office or elsewhere — a tiny instrument can separate inside a canal. It happens. These files are incredibly thin and they work under extreme stress inside curved, narrow canals.
When it does happen, most general dentists don’t have the tools to get them out. That’s where we come in.
Dr. Phan uses the surgical microscope at 25x magnification along with ultrasonic tips specifically designed to vibrate around and loosen separated instruments. Combined with our CBCT 3D imaging, we can see exactly where the fragment is sitting, how deep it goes, and plan the safest approach to remove it.
Not every broken instrument needs to come out. If it’s near the tip of a fully cleaned canal, sometimes leaving it in place and sealing around it is the better call. Dr. Phan will give you his honest assessment — removal when it matters, conservative management when it doesn’t.
If your dentist told you there’s a broken file in your tooth, don’t panic. This is something we handle regularly and most of these teeth can still be saved.
Post and Core Buildup
After a root canal, your tooth needs a solid foundation before your dentist places a crown. That’s where a post and core buildup comes in — especially for teeth that have lost a lot of structure to decay, fractures, or previous dental work.
A post is a small rod (usually fiber or metal) that goes into the treated canal to anchor the core buildup material. Think of it like rebar in concrete — it gives the final crown something strong to grab onto.
When you need a post:
- The tooth has very little natural structure remaining above the gumline
- A front tooth that needs extra support for a crown
- Heavily restored teeth where the walls are thin or missing
When you don’t:
- Plenty of tooth structure remains (most back teeth after root canal)
- The tooth will get a full-coverage crown with good retention
